Your Virtual Visit: Step-by-Step

Here’s how a typical Salubrity Family Health virtual appointment works so you can feel prepared and confident.

Step 1: Receive Secure Link & Complete Forms
Before your appointment, you’ll receive a secure link via email.
This link allows you to complete all required forms such as:
• Medical history
• Consent for treatment
• Release of information (if needed)
Completing these forms ahead of time helps your provider prepare and ensures a smooth visit.
Step 2: Log In & Verify Identity
Use the same secure link to join your telehealth appointment.
Your nurse practitioner may confirm your full name, date of birth, and location (for telehealth compliance).
Step 3: Provider Joins the Session
Your nurse practitioner will join the session, greet you, and review your concerns.
Together, you’ll discuss your symptoms, medical history, and any questions you have.
Step 4: Diagnosis & Care Plan
Based on the evaluation, your provider will give a diagnosis and personalized care plan.
If needed, prescriptions will be sent electronically to your preferred pharmacy.
Step 5: Wrap-Up & Next Steps
You’ll be able to see your visit note through Patient Portal.
